Dosing Guidelines

This page contains general information on common DIY HRT dosing. Always adjust with medical guidance when possible.

Estradiol (Non-Injection)

Form Recommended Dose Range Frequency Notes
Oral Estradiol (17-β) 2–8 mg Daily Lower doses reduce liver strain; higher doses may increase T suppression but carry more risk.
Transdermal Patch 50–200 µg (0.05–0.2 mg) Replace according to package instructions (usually every 3–4 days) Provides steadier blood levels; safer for liver than oral.
Estradiol Gel 1–5 mg applied topically Daily Absorption varies by product and application site, typically 0.75mg/pump

Estradiol Injections

Injectable estradiol comes in several ester forms. Dosage depends on the ester type and vial concentration. A common calculation is: Desired Dose ÷ Vial Strength = Injection Volume. Example: a 5 mg dose drawn from a 40 mg/mL vial → 5 ÷ 40 = 0.125 mL (≈0.13 mL).

Formulation Recommended Dose Injection Volume* Frequency
Estradiol Valerate (EV) 4 mg 0.1 mL (40 mg/mL) Every 5 days
Estradiol Cypionate (EC) 5 mg ≈0.13 mL (40 mg/mL) Every 7 days
Estradiol Enanthate (EEn) 6 mg 0.15 mL (40 mg/mL) Every 7 days
Estradiol Enanthate (EEn) 8 mg 0.2 mL (40 mg/mL) Every 10 days
Estradiol Undecylate (EUn) (MCT Oil) 32 mg 0.3 mL (80 mg/mL) Every 31 days

*Injection volume assumes typical vial concentrations (40 mg/mL or 80 mg/mL). Always confirm your vial strength before calculating your own dose.

Testosterone Blockers

Drug Recommended Dose Frequency Notes
Cyproterone Acetate (oral) 12.5 mg Every 2 days
Spironolactone (oral) 50–200 mg Every day Doses over 100mg are commonly taken in 100mg twice daily.
Bicalutamide (oral) 50 mg Every day
